We are preferred providers for most major insurance carriers including Blue Cross (Anthem), Blue Shield, United HealthCare, Delta, Tricare/TriWest and more. We accept Medicare assignment and are contracted with most workers’ compensation carriers. We are glad to bill your personal auto insurance carrier when Med Pay is available. We DO NOT accept liens. If you are unsure if you are in-network with our office, please contact your insurance directly, as it is plan specific.

It is the responsibility of the patient to know their insurance policy(s). Your insurance policy is a contract between you and your insurance carrier(s). If you have questions about how your insurance processes your claims, or denied treatment, we encourage you to contact your insurance carrier(s) directly. It is your responsibility to review your insurance information prior to your physical therapy treatment. As a courtesy, we will obtain your physical therapy benefits and provide you with an explanation of the coverage quoted to us by your insurance carrier(s). Any information provided to you is an estimate, which has been provided to our office by your insurance company.

You are not in my provider network. What are my options?
  • Many insurance companies have benefits for out-of-network providers. Talk with your insurance company about your policy to see if you have out-of-network benefits.
  • Take advantage of our private pay option.

Will you bill my insurance?

As a courtesy to our patients, we will bill up to 3 insurance companies.

When is payment due?

Co-Payments & Deductibles will be collected prior to each visit.
